That sounds obvious, but it isn't. Most med spas are not set up to accept HSA/FSA cards, which leaves you paying out of pocket and chasing reimbursement on your own. Cosmetic vs. Medical: the line that decides eligibility
HSA and FSA funds are for qualified medical expenses, care that treats or prevents a medical condition. That's the whole test.
- If Botox is smoothing wrinkles, it's cosmetic → not eligible.
- If Botox is treating a condition, jaw pain, migraines, excessive sweating, chronic muscle tension, it's medical → eligible.
The same product (Botox or Dysport) can be either. What matters is the reason it's being used and that it's documented as a medical treatment.
Which Botox treatments qualify?
These are the medical, function-first treatments we perform that are typically HSA/FSA eligible:

- Chronic migraine prevention, Botox is FDA-approved for chronic migraines (15+ headache days a month).
- Hyperhidrosis (excessive sweating), FDA-approved for underarm sweating, and used for hands and feet.
- Trap Tox for chronic pain, trapezius Botox, when it's treating chronic neck and shoulder pain or tension headaches (not purely for shoulder slimming).
What doesn't qualify: cosmetic wrinkle Botox, lip flips, and "Barbie Botox" done purely for aesthetic slimming. If there's no medical condition being treated, it's not an eligible expense.
HSA vs. FSA, the quick difference
Both let you spend pre-tax dollars on qualified medical care. The practical differences:
- HSA (Health Savings Account): paired with a high-deductible health plan, the money rolls over year to year and is yours to keep.
- FSA (Flexible Spending Account): employer-sponsored, and funds typically must be used within the plan year ("use it or lose it").
For eligible medical Botox, both work the same way at checkout.
